Tucson Care Management provides services for people struggling with aging family members. We can help you with the following:

• Assessments and evaluations in home, hospitals, and other
• Development of an individualized comprehensive care plan
• All aspects of an elder's needs are incorproated into a day to day plan
• Ongoing updates and plan revisions considering actvities of daily living,medical treatment, family needs and involvement, fiduciary issues
• Coordination of needed services with all health care providers and others i.e. medical and physician referrals, RN medication monitoring and follow up, pharmacy and drug representative assistance, work with rehabilitation personnel, elder law attorneys, senior care residences, elder home placements, home health care, senior transport, elder physical therapy programs, elder exercise activities
• Communication in written form and telephonic with all persons involved in the elder medical care plan
• Ongoing monitoring of elder's health and situational status changing the care plan as frequently as necessary to continually provide quality care
• Resource information related to disease, medications, nutrition, diet and exercise, aging, dementia, family dynamics, alternate living needs, hospice available
• Fiduciary services provided. Medical, Durable, and Financial POA
• Support and recommendatioins for elder care planning to achieve and ensure an optimal quality of life.
